Our approach to compliance includes key components such as secure, encrypted platforms for call documentation and clinical records, which are essential for effective clinical triage support and nurse consultation. We implement role-based access controls to limit PHI access to authorized personnel only, ensuring that sensitive information is protected during nurse triage. Our secure authentication and audit capabilities further enhance our privacy standards, alongside policies governing minimum necessary access to PHI. Nurse-Led Access to PHI

Only licensed Registered Nurses and authorized personnel involved in patient care have access to PHI. Access is strictly limited to what is necessary to safely triage, assess, and support the patient through our 24/7 healthcare coverage.

All patient interactions are documented within HIPAA-compliant systems to support 24/7 healthcare coverage. Designated members of each practice are granted secure portal access to review: 


- Triaged calls 

- Clinical documentation 

- Selected dispositions 

- Time stamps and workflow details 


This process ensures transparency, accountability, and continuity of care while maintaining strict privacy controls, complemented by clinical triage support and Nurse consultation through effective Nurse triage.

Workforce Training & Accountability

All staff supporting Continuum Clinical Partners’ services, including those providing 24/7 healthcare coverage, receive training on HIPAA privacy and security requirements. This training covers the appropriate handling of PHI, confidential communication practices, secure system usage, and incident awareness and reporting. Additionally, our clinical triage support team, which includes Nurse consultation and Nurse triage, follows internal policies that reinforce compliance expectations and professional accountability.

Business Associate Relationships

Continuum Clinical Partners operates as a Business Associate to the healthcare organizations we support, entering into Business Associate Agreements (BAAs) as required. These agreements outline responsibilities related to the use, disclosure, and safeguarding of PHI while ensuring 24/7 healthcare coverage, clinical triage support, and facilitating Nurse consultation and Nurse triage to enhance patient care.
